Needs Assessment are designed for Customers who are assessing Seeker's social needs, in order to understand how to best help them.
Assessments are visible on the homepage of the site, typically with a "Start a Form" button.
Depending on the site, the Assessment will either be visible without login, or when a Site Worker logs in to their site.
Filling out Assessments
Once you start an Assessment, fill out the questions on behalf of who you are helping. Once complete, you can review the Assessment responses, confirm consent, and submit the Assessment.
Once you submit an Assessment, you cannot edit the responses. However, you can undo the entire Assessment and start over.
What Happens Next
Depending on the Assessment, you may be taken to Personalized Search Results or a Program Recommendation Summary (PRS) to find programs to meet those needs. A banner for the person you are helping will display at the top. When that banner is present, any referrals that you make will be autofilled with that person's contact information. To navigate on behalf of someone else, click "End Session".
